| Membership
is for any individual, firm or corporation engaged in the
production and/or placement of hot-mix asphalt for profit. This is
generally paving contractors and plant operators. In the event
hot-mix asphalt is obtained from another mixture producer, the
liquid assessment is the responsibility of that producer(s).
The dues for this category
are:

plus per ton charge for liquid asphalt cement used in asphalt
hot-mix production |

Major
Refiner |
| Membership
is for any firm which is a basic refiner or an operator of a
storage facility for asphalt cement. The annual dues are $5000. |

Supporting
Associate |
| Membership
is for an individual or firm which is a basic manufacturer or
distributor of equipment or supplies or produces aggregate
materials to be used in the production or transportation of
hot-mix asphalt. The annual dues are $500. |
|
Sustaining
Associate |
| Membership
is for an individual or firm which supports the hot-mix asphalt
industry but does not sell a product or service used directly in
the production, placement or transportation of hot-mix asphalt.
The annual dues are $100. |

| Instructions
for completing the application for membership and dues requirement
are as follows: |

- Print the application
- Complete the top portion of the
application with correct information.
- Determine the appropriate
membership category and check in the space provided.
- Complete your check in the
correct amount, sign the form and return to DAPA.
- If you have any problems with
downloading the application, please e-mail
the DAPA office to have them send you a copy of the
application.
